ID fraud affidavit

The Federal Trade Commission has created an ID fraud affidavit to help victims report fraud to creditors. Instead of filling out several documents for each creditor, victims can fill out one document for all creditors.

FREE CREDIT REPORT

Obtain a free credit report if you have been a victim of identity theft. As of December 1, 2004, under the Fair and Accurate Credit Transactions Act (FACT Act) , you can request and obtain a free credit report once every 12 months from each of the three nationwide consumer credit reporting companies: Equifax, Experian and TransUnion.

If you have already obtained your free credit report within a 12 months period, you may visit one of the links below to order another credit report.

Equifax - Call 1-800-685-1111.
Trans Union - Call 1-800-916-8800.
