- Federal Trade Commission (FTC) ID
Fraud Affidavit
The Federal Trade Commission has created an ID fraud affidavit to help victims report fraud to creditors.
Instead of filling out several documents for each financial institution/creditor, victims
can fill out one document for all financial institutions/creditors. This Fraud Affidavit should only be used if credit or some other funds transfer has already been obtained fraudulently in your name.
