Based on the structure of your business, the following documentation will be required to open your account.
1. Authorized Signer Information (Required for Each Signer)
Each authorized signer must provide:
- Full legal name (as shown on government-issued ID)
- Date of birth
- Social Security Number (SSN) or ITIN
- Residential address (no P.O. Boxes)
- Phone number
- Email address
- Title or role within the business
- Color copy of a valid, unexpired government-issued photo ID
Note: Information must be consistent with submitted business records.
2. Beneficial Ownership & Control Person
Must be provided for:
Ownership Threshold
- Individuals who own 25% or more of the business
Control Person
- At least one individual with significant responsibility for managing the business (e.g., CEO, Managing Member, President)
Each person must provide:
- Full legal name
- Date of birth
- Residential address
- SSN or ITIN
- Government-issued photo ID
3. Business Tax Identification
One of the following is required:
- Employer Identification Number (EIN)
- Social Security Number (SSN) - sole proprietors only
- ITIN (if applicable)
Note: EIN must be supported by IRS-issued documentation or official confirmation.
4. Business Profile Information (Required for All Accounts)
Provide a brief business overview:
- Nature of business (products or services offered)
- Industry type
- Physical business address (U.S. location required)
- Expected monthly deposits and withdrawals
- Estimated annual revenue
- Source of funds
5. Required Business Documentation (By Entity Type)
Sole Proprietorship
- Business License (if applicable)
- DBA / Fictitious Business Name Statement (if applicable)
- EIN confirmation letter (if applicable)
General Partnership
- Partnership Agreement
- EIN confirmation or IRS documentation
- DBA / Assumed Name Filing (if applicable)
- Business License (if applicable)
Limited Partnership / LLP
- Certificate of Limited Partnership (or equivalent state filing)
- Partnership Agreement (if available)
- LLP Registration or Election (if applicable)
- EIN documentation
- DBA Filing (if applicable)
Limited Liability Company (LLC)
- Articles of Organization or Certificate of Formation (file-stamped)
- Operating Agreement (showing authority to act)
- Secretary of State Entity Status or Registration (if available)
- EIN documentation
- DBA Filing (if applicable)
- Authorization document (resolution, written consent, or meeting minutes if applicable)
Corporation (C-Corp / S-Corp)
- Articles of Incorporation (file-stamped)
- Corporate Bylaws
- Corporate Resolution authorizing account opening
- Meeting Minutes approving resolution
- EIN documentation
- Certificate of Good Standing (if requested or required based on entity age/state rules)
Non-Profit Organization
- Articles of Incorporation
- Bylaws or Charter
- Board Resolution authorizing account opening
- Meeting Minutes
- IRS Determination Letter (501(c) status, if applicable)
- EIN documentation
Unincorporated Association / HOA / PTA / Club
- Articles of Association or Bylaws
- EIN documentation
- DBA / Fictitious Name Filing (if applicable)
- Certificate of Formation / Registration (if applicable)
- Governance document identifying authorized representatives
6. Additional Compliance Requirements (As Needed)
The credit union may request:
- Proof of business address (utility bill, lease, or official registration)
- Proof of business activity (contracts, invoices, website, or bank statements)
- Secretary of State verification
- Certificate of Good Standing
- Additional identity verification documents
- Copy of a valid, unexpired Contractor License Issued by the State of California
7. New Business Requirement (Under 2 Years)
For businesses operating less than two (2) years:
- Resume or professional background summary for each owner or principal may be required
8. Data Consistency Requirement
All submitted documentation must:
- Be accurate and verifiable
- Be consistent across filings to a reasonable extent
- Be current and unexpired where applicable
- Clearly identify the legal business entity

Failure to provide complete or verifiable documentation may result in:
- Processing delays
- Requests for additional information
- Denial of account opening following compliance review
